In a ruthless day of violence, the Marsigliese orders shootings of rival Mafia Dons across Naples to seize control. Meanwhile, Perlante barely survives when Alfredo triggers a bomb under his bed, killing Alfredo and Perlante's mistress. The escalating killings push Luca further toward action.

Luca and Perlante attend a meeting at a local soccer stadium where the Marsigliese discusses merging their criminal concerns. Luca argues the merger would funnel drugs and profits to the French boss, alienating their own crews. The talk underscores the stakes of loyalty in the Naples underworld.

A police sweep nets many smugglers across the Neapolitan bay, but Scherino saves Luca from a raid by proposing an alliance to take down the Marsigliese. The two sides briefly unite against a common foe, signaling a shifting balance of power.

In a final scene, Captain Tarantino thanks Morrone at a wharf, suspects the old Don's truth, and lets him go.
