Athira, Bhama, a first-year medical student, is pushed to death by a notorious gang of five friends — Joe Joseph, Govind Padmasoorya; Anand; Anu Ramachandran, Sandhya; Rakhi, Dhanya Mary Varghese; and Satheesh — who are the son of a powerful state minister, Ramakrishnan. Yet their influence and a college principal’s false testimony keep them free to continue their studies.
After the opening titles, the story shifts to a fresh year where the same gang continues its menacing ways. Rohit Menon, Indrajith Sukumaran, arrives from Bangalore to undertake House Surgency in the same college and immediately clashes with the group from day one. The friends hatch plan after plan to corner him and push him out, but each scheme backfires, and a larger collision looms. Then something goes radically wrong: Rohit is killed, and the gang hides his body in a forest. Commissioner Sudeep Hariharan arrives, a relentless figure who is determined to crack the case, and the pressure mounts as the investigation unfolds.
One by one, Rohit’s supposed spirit targets the gang, and deaths follow in chilling, seemingly orchestrated patterns. Joe Joseph is found dead in an anatomy lab, the room echoing with a grim hanging; Rakhi is beaten to death in a forest; Anand is tied up and run over by a truck on a road; Anu is killed inside a hospital with a scalpel. Satheesh is initially suspected and arrested for Rohit’s murder but is released on bail after a police officer—who is his ally—provides false evidence. That night, Sudeep corners the gang and warns that Satheesh’s fate will mirror the others if he continues down this path.
As the case twists through its labyrinth, Sudeep uncovers a startling truth: Rohit is alive. He arrests him from Satheesh’s house, where Rohit had been hiding with a plan to kill Satheesh. On questioning, Rohit reveals that his real name is Ananthakrishnan and that the “Rohit” who died six months ago was a different identity altogether. He admits that he had murdered Satheesh’s friends on purpose, a calculated move to draw the gang into a trap of fear and guilt. Digging deeper, Sudeep learns that Athira was actually Ananthakrishnan’s love interest; on the night of her death, she was on the phone with Ananthakrishnan, and he heard her last breath. From that moment, his mind was set on revenge, a plan that spiraled into a deadly game.
Sudeep’s understanding of Rohit’s pain clashes with Rohit’s thirst for justice, and the detective works to guide the pursuit toward a just resolution, even as Rohit vows to kill Satheesh. The next day, Ananthakrishnan is brought to Satheesh by a policeman who had been Satheesh’s right-hand man. After a prolonged confrontation, Ananthakrishnan, ignoring Sudeep’s cautions, strangles Satheesh with his handcuffs and hurls him from a bridge. Sudeep and his team watch in helpless silence as the act unfolds, and, after a final, fateful confession, Ananthakrishnan surrenders to the authorities, convinced that his long arc of vengeance has reached its end.
