Uzumaki follows Kirie Goshima, a high-school student, and her boyfriend Shuichi Saito, as well as the residents of the quiet Japanese town of Kurouzu-cho, a place that gradually becomes enveloped by a creeping spiral curse. What starts as a strange fascination with coiling patterns soon seeps into daily life, changing how people think and behave. The town’s ordinary rhythms are pierced by an obsession with spirals, turning once ordinary moments into eerie, persistent preoccupations.

As the spiral mystery intensifies, the curse roams through the community in unnerving ways. Citizens grow obsessed with spirals or become paranoid about them, and the effects rip through bodies and minds. There are grotesque transformations that unfold before the town’s eyes: pregnant women behaving like mosquitoes, the dead rising, and many townspeople mutating into winding, snail-like forms. The curse also seems tied to the environment itself, conjuring whirlwinds and whirlpools, and even turning the abandoned lighthouse into a burning furnace while those who gaze at it are drawn into walking in circles. The sense that the spiral is watching, calculating, and expanding becomes impossible to ignore.

Shuichi experiences the toll of these events in a deeply personal way. He becomes reclusive after the deaths of his parents—his father, Toshio Saito, among them—yet he discovers a troubling ability: he can sense when the spiral curse is taking hold, a knack that others routinely dismiss until the next round of paranormal happenings proves him right. The strain of loss and isolation amplifies the town’s fear, and Kirie herself comes under the curse’s gaze in a unsettling turn of fate: her hair begins to curl into an unnatural spiral, draining her life energy and hypnotizing those around her, even threatening to suffocate her when she tries to cut it away. In a bid to outpace the spiral, Kirie and Shuichi rally the townspeople to flee Kurouzu, but their plan falters and their escape proves futile. When they return, they discover that many years have elapsed in what seems like a single, inexorable march away from the spiral. The once-separate row houses have expanded and connected into a labyrinthine whole, a vast structure built by the town’s mutated inhabitants who twist themselves into spirals.

Kirie and Shuichi brave the maze in search of Kirie’s parents, pressing onward through days of wandering until they arrive at Dragonfly Pond, which drains away to reveal a spiral staircase descending into the earth. As they descend, Shuichi is hurled into a pit by a mutated citizen, and Kirie follows, slipping and falling but eventually surviving as the ground below becomes a sprawling, ancient city made entirely of spirals. Inside this otherworldly center, she encounters petrified townspeople, including her twisted and stone-like parents, along with many others who share the same fate. The city itself seems to possess a will of its own, with the spirals surrounding them acting as a sentient force.

The path leads to a stark realization: the center’s spirals seem to govern the world outside, and a stone tower—sharp and drill-bit shaped—rises from the city, becoming the centerpiece that breaches the surface. As Kirie and Shuichi cling to each other, their bodies twist and intertwine, symbolizing their acceptance of the never-ending cycle. The moment suggests that the spiral’s curse began with the same instant that it ends, for time accelerates away from the center while it freezes at the heart. The spiral’s power endures beyond the town, implying that the events will repeat whenever a new Kurouzu rises from the ground to take its place.
