The film reveals that the deadly game originated from a desperate act of survival at a Mexican mission church, where a young woman named Inez once summoned the trickster demon Calax to kill an abusive priest. To stop the demon’s rampage, Inez trapped him in a pottery jar using a blood sacrifice, but the curse is unleashed in the present day when Sam, a stranger who lured the protagonists into the game, inadvertently destroys the vessel. The survivors learn that the rules are rigid and inescapable; because Sam broke the jar, he is the only person capable of performing the ritual to trap Calax again.
