Having struggled with infertility, Robin and Daniel joyfully adopt Kathelia, a young orphan who doesn't speak. Their happiness soon turns to fear as strange and disturbing events begin to occur, and Kathelia's behavior becomes increasingly unsettling. They suspect a sinister force is at work, threatening their family and forcing them to fight for their survival against a growing darkness.

In the final act Daniel finally follows the address Kathelia hands him to, a decaying house where the previous foster parents are already being consumed by the same black‑mold, flies and rot that have haunted his own home. The elderly woman, Evelyn, is chained and crawling with maggots, a visual echo of the parasite that has been using each caretaker as a doorway. When Daniel escapes with Kathelia, the couple’s own house erupts into chaos: Robin’s “pregnancy” explodes in a bathtub of blood, revealing not a baby but a malformed, lifeless lump that the doctor later identifies as the same dead twin that resides inside Kathelia’s brain.

Desperate to stop the entity once and for all, Daniel, aided by the psychic Fiona, pulls a drill from the garage and drives it into the hollow of Kathelia’s skull. As the drill bites into the frontal lobe, a pale, half‑formed fetus—its eye blinking with a murderous intelligence—pushes its way out. The creature snarls, trying to latch onto Daniel’s wound, but Kathelia, now able to speak for the first time, snarls back and snaps the newborn’s neck, ending the twin’s reign of terror. The act also liberates Kathelia’s voice; she whispers “sister” one last time, confirming that the mute child had been silenced by the parasitic twin all along.

With the twin removed, Robin’s grotesque pregnancy collapses, the swelling in her belly subsides and the jars of “Janice” fetuses lose their power. The immediate danger is over, and the film closes on a quiet, unsettling note: Kathelia, cradled by her father, looks at him with a tentative smile, finally able to communicate, while the house remains littered with the remnants of the horror that has been ripped from its walls. The supernatural threat has been surgically excised, leaving the surviving family to reckon with the trauma—and with the knowledge that a simple drill was enough to end a monster that had lived inside a child’s head.
