After dropping off her son Evan with her ex-husband Cliff, Renee Morgan faces an unimaginable ordeal. During what appears to be a routine roadside tire repair, she is unexpectedly abducted by a group of five individuals, all orchestrated by the sinister Dr. Nyman. In a swift and shocking moment, Renee is stunned with a taser and taken captive in a vehicle, where she is chained and transported to a hidden warehouse laboratory. Upon awakening, she finds herself strapped to a table and held prisoner in a room designed to instill fear.
In this dark place, Renee encounters a fellow prisoner named Seth, who cryptically urges her to remember “G10 12X.” Another captive, Blake, communicates with Renee through a vent from an adjacent chamber, shedding light on their horrifying reality: their captors are subjecting them to relentless tests based on their deepest fears. Furthermore, she learns that this enigmatic group has been monitoring individuals globally for decades, exploiting their vulnerabilities.
Under the watchful eyes of Terrence and subjected to the twisted experiments of Dr. Nyman, Dr. Raxlen, Dianne, Colette, and an ominous figure known as the Bald Man, Renee’s fears manifest in horrifying ways. Having arachnophobia, she panics as Dianne and the Bald Man introduce a spider into her confinement. In a moment of desperation, Renee manages to escape her restraints and navigates the vent systems of the facility.
As she explores the labyrinth of holding cells, she catches glimpses of other captives undergoing tests tailored to their phobias. Eventually, she discovers Blake’s room and hides beneath his table as the doctors enter. In a chilling display, they exploit Blake’s fear of snakes while revealing their unsettling goal: to use terror to provoke a genetic transformation in their subjects. Following Blake’s failed final test, nurses administer a sedative, claiming it will return him home. As the opportunity arises, Renee discreetly slips out into the hallway, determined to discover more about her captors’ intentions.
Acknowledging her potential as the key to their sinister plans, the captors resolve to put Renee through one last trial. She quickly retreats back to her room, just in time to avoid detection. Dianne, employing a deceptive charm, touches Renee and speaks about her son, Evan, claiming she can extract information about him through her skin. She further reveals that “G10 12X” refers to a genetic code encoded in all humans. By instilling terror in their subjects, they earn the ability to alter human genetics and facilitate evolution.
Proving her point, Dianne grotesquely morphs her head, unbeknownst to Renee who stealthily seizes the syringe that was meant for her. In a moment of defiance, Renee stabs Dianne and makes her escape into the heart of the facility. Here, she uncovers a laboratory containing DNA samples from various subjects and overhears a conversation about her ovum being developed to accommodate non-human sperm.
However, her escape is short-lived when the Bald Man, sensing her presence, opts against chasing her and instead isolates her using the facility’s technology. Captured once more, Renee awakens to a horrifying revelation: Terrence, Dr. Nyman, Dianne, and the Bald Man showcase her newly enhanced physical abilities, claiming that by overcoming fear, they have transcended humanity. They now intend to reshape humanity by evolving their gene pool.
To further their experiments, Renee is strapped into a transparent helmet, besieged by spiders crawling on her face. In a frenzy, she fights against the mask, and at that moment, her fear dissipates. As she undergoes a transformative rupture, her head distorts grotesquely, ultimately breaking free from her restraints.
Returning to her life under the guise of allegiance to her captors, Renee attempts to persuade Evan that she knows individuals who can assist him with his focus challenges; however, Evan remains unconvinced. The situation takes a grave turn when Terrence, the Bald Man, and another accomplice confront Renee at home, intent on forcibly taking Evan. In a dramatic twist, Renee experiences a change of heart, urging Evan to escape, which he successfully does. Terrence, undeterred, informs Renee that their search for her has spanned a significant time, and now they believe they have aligned their plans perfectly.
