Prakashan [Fahadh Faasil] yearns to travel abroad, especially to Germany, and in a bid to sound more modern, he adopts the name P. R. Akash. Though trained as a nurse, he views the nursing field as low paid and thankless, and he harbors a sexist bias about the profession, certain it is better suited for women. His plan to escape this reality centers on going abroad by marrying a foreign citizen to secure a family visa, with the intention of dumping her afterward and living his life unchecked.

To reboot his dreams, he rekindles an old flame with Salomi [Nikhila Vimal], a nurse who is herself on the cusp of migrating abroad for higher wages. He imagines Germany as a bright horizon, and begins to learn German in hopes of making that dream real. He confides in his local guardian Gopalji [Sreenivasan], a former student of his father, that his affection for Salomi was largely a ploy to gain a visa under the emigration rules. Yet when Prakashan visits again, he discovers that the visa process still demands a substantial sum of money.

Facing this barrier, he makes drastic moves: he sells his motorbike and convinces Gopalji to steal a necklace from his wife, pulling the old man into police trouble. Ironically, Salomi manipulates him and his family, using them as stepping stones to secure money and a path to Germany. She ultimately cheats him, marrying a German citizen to stay in the country, and Prakashan realizes that the visa scheme was a trap. He abandons the plan and remains in Kerala, choosing to confront his consequences rather than chase a distant dream.

Back in Kerala, Prakashan begins to repay his debts through hard labor for Gopalji and eventually finds work as a home nurse. His life takes a sharper turn when he accepts a job with Tina Mol [Devika Sanjay], a sharp, jet‑setting woman who loves junk food and lives in a world of privilege. Despite an initial clash — she even lets her dog loose on him when he won’t let her friend Shruthi deliver a burger — a genuine bond gradually forms between them. In time, Prakashan also develops a romantic interest in Shruthi [Anju Kurian], adding a new layer of complexity to his moral awakening.

During a deeply emotional moment, Tina confides that she longs to meet her father before she dies. Prakashan reassures her that she has her whole life ahead, only to learn that she is terminally ill with a malignant brain cyst. After she bleeds from the ear and undergoes treatment, he recounts a story from America about a terminal cancer patient who was cured by laughter and living fully. He embraces this philosophy, inviting Tina’s schoolmates over, organizing badminton games, and trying to fill her days with joy rather than fear.

Tragically, Tina passes away during one of his stories, a loss that marks a turning point for Prakashan. Her death becomes a catalyst for a profound transformation: he shifts from chasing a paycheck-driven career to becoming a compassionate, grounded nursing professional who sees nursing as more than money — it is a calling to help people in need.
