As a final act of humiliation, they upload the footage of their sexual encounter to the internet, ensuring his private indiscretion becomes a public spectacle. The film closes with the women driving away in Evan's car, leaving him trapped and helpless, destined to be discovered by his wife in a state of utter ruin. This ending emphasizes the total destruction of Evan's life—not just his fidelity, but his family, his career, and his dignity.

The ambiguity of this finale is deliberate, forcing the audience to sit with the uncomfortable reality that the "villains" win. Throughout the film, there are hints that Genesis and Bel are not simply sadists but perhaps victims of past trauma acting out a twisted form of vigilante justice against unfaithful fathers. However, the theatrical release offers no explicit explanation for their actions, leaving their motives shrouded in mystery. By denying Evan a chance to fight back or understand why he was targeted, the movie mirrors the chaotic and senseless nature of the violation he endured. There was, however, an alternate ending filmed that shifts the narrative perspective significantly. In this version, the women steal Evan's family dog before departing. Utilizing a GPS tracker on the dog's collar, a determined Evan locates their residence. He arrives at their door and knocks, to which they respond with the film's titular line, "Who's there?" This conclusion flips the script, transforming the story from a psychological thriller into a potential revenge tale. It suggests a sequel where Evan tracks down his tormentors, potentially allowing the audience to learn the backstory of Genesis and Bel, and offering a more traditional catharsis where the victim regains control.

Despite the appeal of seeing Evan get his revenge, the theatrical ending is arguably the more fitting conclusion for the film's specific tone. Knock Knock operates on a level of dark absurdity and uncomfortable comedy, and having Evan remain the victim serves the story's satirical elements better. If he were to immediately switch roles to an avenging hero, it might simplify the complex power dynamics the movie established. Ultimately, the theatrical ending leaves a lasting, unsettling impression, cementing the film as a story about consequences and the terrifying volatility of unchecked desire, rather than a straightforward revenge fantasy.
