The Green Mile concludes with one of the most heartbreaking yet thematically rich endings in modern cinema. John Coffey, a gentle giant possessing miraculous healing powers, accepts his execution despite being innocent of the crimes for which he was convicted. While he possesses the strength to escape and the supernatural abilities to resist, Coffey chooses to die because he is exhausted by the cruelty of the world. He describes himself as being tired of the pain and malice he feels constantly, viewing death as a mercy rather than a punishment. Before his death, he passes a portion of his gift to prison guard Paul Edgecomb, an act that changes Paul's life forever. In his final moments, Coffey smiles—a gesture meant to comfort Paul and signal his trust that the guard will use this inherited power for good.

The film's epilogue reveals the true cost of this miraculous gift. Paul is shown years later as an elderly man, revealing that he is 108 years old. Using Mr. Jingles, a mouse healed by Coffey that has lived for decades, as a reference point, the film implies that Paul may live for hundreds of years. However, Paul does not view this extended existence as a blessing. He sees his longevity as a punishment for his role in the execution of an innocent, God-like figure.
