In the film’s climax the truth behind the “possessions” finally surfaces: Dr. Shruti Rawat had arranged for the heart of a comatose patient, Amy R. Mathews, to be transplanted into Anjali Malhotra—the woman who now calls herself Mary Mathews. The illicit operation was carried out without consent, and Shruti’s decision to pull the plug on Amy Mathews set off a chain of guilt‑driven hauntings that manifested as the uncanny, cross‑shaped phenomena throughout the story.

When Gajraj Awasthi pieces together the hospital records, he discovers that the supernatural attacks were a violent expression of the transplanted heart’s “memory” and of Shruti’s own buried remorse. Confronted with the evidence, Dr. Aditya Rawat—who had been complicit in covering up the crime—cannot escape the fallout. Both doctors are arrested for their role in the illegal transplant and the resulting deaths.

During her brief imprisonment, Shruti goes into labour. She delivers the baby in the hospital, but the child is taken into state custody. Gajraj, who has long wanted a second child, decides to adopt the newborn, promising the child a stable home with his own wife.

The final scene returns to Gajraj speaking with his mentor about the case’s closure. The camera slowly reveals that the mentor is merely a figment of Gajraj’s imagination—a hallucination born of the trauma he endured while confronting the darkness hidden in his own town. The imagined mentor fades away as Gajraj finally finds the closure he needed, hinting that the mind can conjure its own “supernatural” comforts when reality becomes unbearable.
