After casting painter‑video artist Mania Akbari as the protagonist of his experimental Ten (2002) and seeing her direct 20 Fingers (2004), Abbas Kiarostami encouraged her to make a sequel. In 10 + 4, Mania battles cancer, having undergone surgery, lost her hair to chemotherapy and stopped wearing the compulsory headscarf. The film follows her candid conversations with friends and family, moving from the gondola of her first feature to a hospital bed.
