Tomie: Replay

Doctors rush a six-year-old girl into the ER for an emergency operation. They discover a disembodied head named Tomie inside the girl's abdomen, alive and growing. Tomie is sealed in an alkaline tank in the hospital basement as investigators scramble to understand the bizarre phenomenon. Meanwhile, five hospital staff who were present during the operation vanish without a trace.

A few nights later, Takeshi visits his sick friend Fumihito in a hospital room. A naked, fully grown Tomie appears and begs to be taken out. Takeshi escorts Tomie to his apartment, leaving Fumihito behind without explanation, already under Tomie's sinister influence.

The next morning, Yumi, Morita's daughter, searches for her missing father at the hospital and meets Dr Tachibana, the only doctor who hasn't fled. Tachibana gives her a journal written by her father, and soon afterward he kills himself. The journal reveals that Tomie’s blood infected Morita and another doctor, causing regeneration and insanity.

At a party, Yumi and Fumihito realize they are both searching for the same girl named Tomie. They decide to join forces to uncover what happened to Yumi's father and Takeshi. Their alliance shifts the pursuit from individual actions to a coordinated investigation.

The six-year-old girl who underwent the operation had previously received a kidney transplant from another Tomie. From this kidney, Tomie begins regenerating inside her body, foreshadowing future horrors. This backstory explains how Tomie can reappear in different bodies.

Takeshi kills and decapitates Tomie out of jealousy, only to watch her regenerate anew. The horrific event drives Takeshi mad and lands him in a mental hospital, while Fumihito becomes Tomie's new target. Tomie's resilience fuels the cycle of possession and murder.

With Tomie continuing to manipulate minds, Fumihito becomes her new prey and starts acting under her influence. His allegiance to Tomie grows as she regrows in new bodies and lures others into danger. The line between victim and puppet begins to blur.

That night Yumi's missing father has a brief, cryptic encounter with her, mumbling about needing to kill Tomie. The following day, his bloated and deformed body is found in the hospital basement, confirming the supernatural danger surrounding Tomie. The mystery deepens as the family grieves.

At the father's funeral, Yumi receives a note from Tomie inviting her to meet that night at the hospital. The message signals a looming confrontation and reveals Tomie's ongoing reach beyond death. Yumi is left to prepare for a final encounter.

Yumi arrives at the hospital and Tomie taunts her, while Fumihito, still under Tomie's influence, appears to confront Yumi. The scene gathers the main players for a decisive showdown under the hospital lights. Loyalties and reality blur as the confrontation escalates.

In a last act of defiance against Tomie, Fumihito decides to kill her instead of Yumi. He decapitates Tomie and burns the remains, ending her immediate threat. Yumi and Fumihito escape from the hospital, leaving the nightmare behind for now.
