David Chamberlain, an anxious soon-to-be father, is racing to the hospital where his wife is in labor. His routine drive takes a terrifying turn when a mysterious man, known only as The Passenger, enters the back seat. With a gun pointed at David’s head, the Passenger forces him to pick a card, leading David to randomly select the Ace of Spades—a card the Passenger conveniently reveals he had in his pocket all along. Driven by fear for his life, David complies and begins an unpredictable road trip filled with dread and desperation.

Throughout their harrowing journey, the Passenger remains elusive, only mocking David’s anguished attempts to appeal to his sense of humanity and discuss his family life. As they travel, David makes several frantic attempts to escape. He tries signaling for help to a woman at a gas station, but she drives away oblivious to his silent plea. His next escape effort involves trying to attract the attention of a police officer, but the Passenger escalates the situation violently, turning their routine stop into a nightmare when he angrily confronts the cop before brutally shooting him multiple times.

The Passenger, who appears to know much more about David than he lets on, continues to taunt him, hinting at their grim destination. During one particularly reckless moment, David leaps from the moving vehicle, prompting a dangerous crash as the Passenger struggles for control. However, the fear of abandoning his wife, who remains in the hospital, brings David back to the car, forcing him to continue this perilous ride.

Their next stop is a diner, where the Passenger begins to unveil the twisted backstory behind their grim encounter. He recounts a tragic tale about a bookkeeper associated with an Irish Mob boss named Jacob Sullivan from Boston. As the story unfolds, it becomes chillingly clear that David is not simply a random victim; instead, he is intricately linked to the Passenger’s past calamities. The recounting takes a darker turn as the Passenger explains that, just like the bookkeeper who was lured to his demise, David is also marked for death upon reaching their final destination.

The tension escalates as the Passenger, increasingly unstable, confronts David about his deceptive life. In a frenzy, he resorts to violence, attacking the diner and setting it ablaze while David desperately calls for help and flees outside. Yet, even in the parking lot, the Passenger continues to unravel more of his story, revealing his identity as the bookkeeper, whose very existence was shattered by Sullivan’s brutal actions.

As they struggle for dominance, the depth of their intertwined fates is laid bare. The Passenger accuses David, now realized as James Levine, of complicity in the murder of his wife and daughter, insisting that David must acknowledge his culpability. Faced with the weight of the truth, James discloses that Sullivan orchestrated the hit, leading him to take drastic measures—swerving off the road, crashing the car, and fatally wounding the Passenger in the process.

In their final confrontation, the shadows of betrayal and guilt culminate in a violent end, as David, now stripped of any remaining pretenses, confesses his role in the Passenger’s family tragedy. With the Passenger mortally wounded, David chooses to seal his fate by strangling him, the act driven by both malice and a profound sense of inevitability.

As sirens approach, James cleverly wipes his fingerprints from the murder weapon, placing it in the Passenger’s grasp to frame him for the police officer’s death. Among the wreckage, he discovers the Ace of Spades card, a haunting reminder of their twisted encounter. Moments later, a voicemail reveals the birth of his daughter, a child who bears his eyes, igniting a twisted smile on his face as he stands, ready to greet the approaching law enforcement, masking the horrifying truth behind his facade of innocence.
