A legal deposition is arranged so Jane can recount the events and explain her actions. She narrates the surgery on Frank and the murders at her clinic, yet she shows no genuine remorse. The deposition becomes a platform for her justification rather than contrition.

Fueled by revenge, Frank sets out to kill everyone connected to his operation, including the men on Honest John's payroll. He methodically targets those involved, aiming to erase all traces of the past. His campaign escalates toward a final confrontation.

Frank interrogates Honest John to learn the doctor's identity and cuts through lies. Honest John implies that Johnnie knows the doctor and may be involved. Frank then kills Honest John and moves on to confront Johnnie.

Johnnie explains she was hired to monitor Frank and acted out of fear for her own safety. She admits growing feelings for Frank and agrees to help him lay a trap for the doctor. She decides to stay in Reno as part of the outcome.

The doctor's guards anticipate Frank, and he is sedated after a staged confrontation. He wakes in a straitjacket to face Jane, who intends to amputate his right arm to stop him. The confrontation escalates into a brutal struggle.

Frank overpowers guards and fights Jane, who weaponizes fear and a knife. He shoots Jane non-fatally to halt her, then stages the scene to imply her assistant killed everyone. After securing his revenge, he records a video to tell his side of the story for release after his death.

In the final present-day sequence, Jane meets Galen and reveals her attempts to be deemed mentally fit for trial, ultimately accepting her fate. The deposition serves to tell her side, while Frank's actions loom over the narrative. The film closes with Jane examining her mutilated hands, the result of Frank severing her fingers to ensure she cannot practice again.
