Throughout Annabelle: Creation, the story reveals how the doll came to be, linked to a personal tragedy for her creator, Samuel Mullins. Following the death of his daughter Bee, who was beloved and named Annabelle, Mullins creates this doll, which becomes the vessel for a malicious demon called the Ram. Initially, the spirit manipulates the Mullins family through the haunted doll, trying to possess the daughter Esther and later, other girls in the orphanage, as it seeks a physical form. The film culminates with the spirit taking over Janice, a polio-stricken girl, and the doll seemingly returning to a malevolent state, as the spirit’s influence persists.

The ending connects directly to the original Annabelle film, which depicts the doll’s possession of Mia and its attempts to harm her and her baby. The final scenes of Creation are set in 1969, immediately before the events of the first film, revealing that the doll has re-entered the story and the spirit’s influence remains active. Notably, Mia, played by Annabelle Wallis, is shown briefly in a cameo, tying her directly to the original story and hinting that the spirit’s ultimate goal is to possess her child, which it nearly accomplishes.

A key moment at the end involves the doll in a mid-credits scene, where it suddenly appears to move and turn toward the camera—an unnerving tease that hints the doll or the spirit within it might still be alive, despite claims of being “cleansed.” This suggests a lingering malevolence, possibly indicating that the spirit is not fully gone and that the doll has some form of autonomous evil.
